1 Start 2 Complete Case Study: Shoulder Injury in a College AthletePatient Profile:Jake, a 21-year-old male, presents to the clinic with right shoulder pain that has worsened over the past two weeks. He is a college baseball pitcher and reports the pain started after an intense game in which he threw over 100 pitches. Since then, he has experienced intermittent sharp pain along the anterior shoulder, especially during overhead throwing or lifting. He also reports a feeling of "weakness" in his shoulder and occasional clicking when he moves it.Jake has tried icing and resting his shoulder, but his pain persists, particularly when performing overhead activities or while lying on his right side. He denies any previous shoulder injuries or trauma. His primary concern is returning to play as soon as possible, as the baseball season is nearing its peak.Physical Exam Findings:Inspection: No visible deformities, no muscle atrophy.Palpation: Tenderness over the bicipital groove and anterior shoulder, no swelling.Range of Motion (ROM): Active and passive ROM are mildly restricted in abduction and external rotation due to pain. Flexion and internal rotation are within normal limits but provoke discomfort at the end range.Strength: 4/5 strength in external rotation and abduction, pain limiting full effort. No scapular winging observed.Special Tests:Neer Impingement Test: PositiveHawkins-Kennedy Test: PositiveSpeed's Test: PositiveO'Brien’s Test: NegativeEmpty Can Test: PositiveApprehension and Relocation Test: NegativeImaging:Jake’s X-ray of the right shoulder shows no fractures or dislocations. Questions for Case Assessment:Using this information, please answer these questions:Based on the patient's history and physical exam findings, what is your differential diagnosis for Jake's shoulder pain? Which diagnosis do you consider most likely?Consider Jake’s specific sport and activity level, the positive special test findings, and his symptom presentation. Explain the significance of the positive Neer Impingement Test, Hawkins-Kennedy Test, and Speed's Test. What shoulder structures are being assessed by each of these tests?Discuss the pathophysiology that each of these tests helps identify and how they narrow down potential diagnoses. What additional special tests or assessments could be performed to further evaluate Jake's shoulder injury?Identify at least two additional special tests or assessment techniques, and explain what they evaluate and how they would help confirm or rule out other shoulder pathologies. What is your initial management plan for Jake, considering his desire to return to play? What treatment options should be considered, and what modifications to his activity level would you recommend?Provide a management plan that includes both conservative treatment options and criteria for potential referral or surgical intervention based on Jake’s symptoms and exam findings.
