ForwardCare Ecosystem Nurse Care Manager Training
The Care Ecosystem model was originally designed for health care navigators to provide support to persons with dementia and their caregivers. This model has been adapted by an experienced group of nurse care managers and dementia experts for use by iCMP nurse care managers. The following modules present the adapted Care Eco models, serving as the pre-requisite to live trainings that will occur in September and October.

Learning Objectives
- To describe how the Care Eco model supports PWD and caregivers.
- To understand the protocols to be used in a clinical setting to improve clinical assessment and treatment of PWD.
- To define the support strategies and resources available to help reduce caregiver distress related to the behavioral symptoms of dementia.
