MGB West Health Champions 2025-2026
The West Health Champions program will provide participants with education & training in the fundamentals of delivering Age-Friendly care to older patients, focused on the 4Ms of Medication, Mentation, Mobility and What Matters. West Champions will learn quality improvement & leadership techniques to enable the spread of evidence-based geriatric care through MGB.
Target Audience
This activity is intended for PAs and nurses within the Mass General Brigham system.
Learning Objectives
Upon completion of this activity, participants will be able to:
- Differentiate between aging and frailty and strategies to identify and mitigate frailty
- Identify the “current state” for a quality improvement project
- Demonstrate the ability to work with interdisciplinary teams
