The measures that are part of this MOC project are part of the community incentive program, which this year is part of the larger IPF quality slate, but are two separate funding/incentive structures.
Practices will choose from a menu of measures and implement individual projects. The practices will send their practice-specific project information to QPE. Each practice will have their own specific baseline, aim and interventions.
