The goal of this measure is to track and then improve clinician compliance with requirement that encounters and documentation are completed within 6 days of providing care. Outpatient clinicians will be tracked by the department running a query through Epic to determine the percentage of outpatient encounters that were closed within 6 days of the clinical visit over a 1-month timeframe. Inpatient clinicians will be tracked by running a chart audit of 5 charts picked at random.
