MGB Care Continuum & Population Health Services Educational Spring Retreat

The Case Management Educational Spring Retreat is designed to support MGB Case Managers in strengthening how they work both individually and as part of an interprofessional team. This session focuses on practical strategies for improving collaboration, understanding personal and team strengths, thereby enhancing the coordination of care for highly complex patients. Through sessions on team‑based practice, strengths identification, creativity in the workplace, and personal well‑being, participants will gain tools that can be applied directly to their daily work. The goal of this summit is to reinforce the essential role Case Managers play in guiding patients through the care continuum and to support the continued development of a strong, effective team.
Target Audience
This activity is intended for case managers working within Mass General Brigham, including those in the Care Continuum and Population Health Services Organization.
Learning Objectives
Upon completion of this activity, participants will be able to:
- Identify and articulate their personal strengths using the Gallup StrengthsFinder framework to enhance communication, problem‑solving, and patient engagement.
- Apply principles of interprofessional collaboration to improve care transitions for highly complex patients and strengthen team cohesion across the care continuum.
- Integrate strategies for resilience, creativity, and wellness into daily practice to support sustainable, high‑quality case management work.
