MGH Life Cycle of a Safety Report 2025

This activity will be held both virtually & in-person at MGH.

This presentation will provide education to the Department of Pharmacy about the safety reporting process. It will describe the events that occur once a safety report is entered, and how they can lead to system improvements.

Target Audience

This activity is intended for pharmacists & pharmacy technicians.

Learning Objectives

Upon completion of this activity, participants will be able to:

  1. Describe the process of safety reporting & follow up actions to support a patient safety culture.
  2. Review the steps in safety event analysis, including safety review follow up, triaging to other departments and safety organizations, systems & process improvement, and feedback to reporter.
  3. Review a patient case to identify causes, contributing factors, and possible improvement actions.
