Description
The Serious Illness Care Program is a systems-based approach to advance care planning. The goal of the program is to facilitate conversations with patients and families about their values, goals, and preferences regarding future medical care to help ensure that people receive medical care consistent with their wishes. Clinician participation in the Serious Illness Care Program can enhance patients' understanding of their illness and prognosis, allow patients control over their decisions, relieve burdens of decision making on family members, and help patients live as well as possible with serious illness.
Target Audience
This activity is intended for MGH clinicians (physicians, physician assistants, nurse practitioners, social workers, nurses, and others affiliated with MGH).
Learning Objectives
Upon completion of this activity, participants will be able to:
- Identify their role in a system-based approach to improving care for patients with serious illness.
- Practice using the Partners Serious Illness Conversation Guide.
- Differentiate between conversations about decisions and conversations about values.

Upon successful completion of this activity, if you wish to receive MOC (Part 2) points you will need to enter your ABIM ID number in the evaluation section. The Partners Office of Continuing Professional Development will verify completion of your activity and report to the ABIM. Diplomats must check their ABIM Portfolio for confirmation of their MOC points.
ABIM MOC Recognition Statement
“Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 2.25 Medical Knowledge MOC points in the American Board of Internal Medicine's (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity. It is the CME activity provider's responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit."
Nursing Credit
Norman Knight Nursing Center for Clinical & Professional Development at Massachusetts General Hospital is approved as a provider of nursing continuing professional development by the Ohio Nurses Association, an accredited approver by the American Nurses Credentialing Center's Commission on Accreditation (OBN-001-91).
Criteria for successful completion include: check-in at the beginning of the program, attendance at the entire event and submission of a completed evaluation. No unresolved conflict of interest exists for anyone in a position to control the content of this activity.
Social Worker Credit
