After the completion of Epic Provider Training Courses, physicians may now claim your credits and receive your certificate, and other health care providers may receive a participation certificate. 

Click 'Register' to begin the process.

After clicking 'Register', complete the following steps. These steps will be available after registering.

Click 'Next' at the bottom of this page to access the 'Evaluation' form, where you will answer two brief questions. Click 'Submit' when finished.

You will be prompted to indicate the number of hours you participated in the classroom training sessions, and personalization lab from the drop-down. Once selected, click 'Save' and you will be able to view and print your certificate.
