Partners Maintenance of Certification Practice Improvement Projects (MOC Part IV)
MOC Practice Improvement Projects (MOC Part IV)
Partners Office of Continuing Professional Development (CPD) can provide AMA PRA Category 1 CreditsTM and MOC Part IV credit for approved projects.
Although specific requirements vary by specialty board, generally MOC Part IV requires physicians to complete a process improvement project which consists of:
-Identifying a problem
-Reviewing baseline data (personal or practice level data)
-Completing an intervention
-Reviewing post-intervention data
MOC Part IV Physician/Clinician/Participation Attestation
In order to receive MOC Part IV credit, complete the online attestation within one month of receiving the attestation instructions. Attestation instructions will be sent in an email from phsmoc@partners.org
Please complete all required fields in the “Evaluation” (Evaluation form is the same as the attestation form). Please note: If your board has specific criteria claiming MOC credit (e.g. on-line registration, MOC fees, etc) it is your personal responsibility to know and comply with these requirements.
The Partners HealthCare MOC Portfolio Approval program will report your participation on your project to your board.
Instructions for receiving MOC Part IV Credit
1. Click on the Evaluation tab to complete the Physician Participation/Attestation form
2. Please complete all required fields in the Evaluation form
3. Submit completed form
