Technology-Enabled Education for Community Health (TEECH) - Optimizing Care for Older Adults: Applying the Geriatric 4Ms Framework in Primary Care
This course is designed to provide rural health care providers with knowledge of basic tenets of the care of older adults including a framework to assess and manage conditions common in this age group. The course will utilize clinical pearls and evidence-based practice. It will also provide an opportunity for practical application of the learnings with case-based discussions. nd physical health

Target Audience
This activity is intended for tribal- and rural-area primary care clinicians practicing at three primary care sites (Arkansas, Oklahoma, West Virginia) that are a part of a longitudinal tele-education project. There are no external audiences outside of these three sites. The care team includes primary care physicians, nurses, PAs, physical therapists, social workers, community health workers, pharmacists, nutritionists/dieticians, and occupational therapists.
Learning Objectives
Upon completion of this activity, participants will be able to:
- Understand the demographic changes of the population served by primary care providers and introduce a framework, the 4Ms, as a method to understand and prioritize healthcare delivery in this population
- Utilize a novel AI application for real-time case review to gauge understanding and reinforce principles of care
- Develop a structure for care of older adults with delirium and cognitive decline as well as mood disorders including:
- Review of screening tools
- Management principles including discussion of newest therapeutic options
- Discussion of role of multidisciplinary team members in care delivery
- Examine nutritional considerations with impact on promoting brain health
- Recognize the impact of mobility concerns in older adults including:
- Efficacy of various screening instruments to identify those at highest risk
- A toe to head approach to screening
- Evidence based review of impact of physical activity on mitigating fall risk
- Explore role of dietary supplementation including calcium and vitamin D in maintaining bone health
- Discuss the impact of medication prescribing on patient safety and outcomes including:
- Review of physiology of aging and relevance to pharmacotherapy
- Principles of polypharmacy
- Sharing deprescribing framework and resources
- Apply the 4Ms to clinical decision-making and care planning including:
- Review of current communication tools and resources
- Overview of prognostication
- Integrate interdisciplinary approaches to support person-centered care in practice
- Describe the relevance of the 4Ms framework to the Institute for Healthcare Improvement (IHI) age-friendly health systems and value-based care models
- Discussion of pitfalls and opportunities for transitions of care
- Examine resources in post-acute care setting
- Review principles of healthy aging to maintain independence and safety in older age
- Explore impact of socialization, cognitive stimulation, and physical activity in promoting cognitive and physical health
- Learn basic nutritional principles with emphasis of those most relevant to an aging population
