Course Description

Medication reconciliation is the process of compiling an accurate list of a patient’s medications and utilizing this information in treatment planning.  In addition to helping directly prevent medication errors, a comprehensive medication reconciliation can improve quality of care and patient outcomes.

Please join Jonathan Zebrowski, MD, MHQS, to learn the key principles of medication reconciliation at Mass General, including medication review and order reconciliation.  Participants will also review updated inpatient dashboard data and discuss implications for ongoing improvement as well as learn about ongoing work in the ambulatory setting to improve outpatient medication reconciliation processes.


Learning Objectives

Upon completion of this activity, participants will be able to:

  1. Describe the main principles of Medication Review & Order Reconciliation
  2. Discuss updated inpatient dashboard data and implications for ongoing improvement
  3. Describe workflow standardization in the ambulatory setting and how this can help the medication reconciliation process, including during transitions of care

Target Audience

This activity is intended for members of the Quality and Patient Safety Committee (QPSC) and staff they work with who will benefit from the training.
